Set up and opt in to 2FA

The UW will require two-factor authentication (2FA) for your UW NetID account to protect both your and the University’s information. With 2FA, you sign in first with your UW NetID and password, then use a 2FA device (such as your smartphone) to confirm your identity. If you have not set up 2FA, you may not be able to register, and you will not be able to access UW online systems, such as Canvas, Zoom, MyUW and others.

Don’t wait — “lock down your login” today with the Duo Mobile app for 2FA.

Posthumous Degree Request Form

Undergraduate and Professional Schools – UW Seattle

This form accommodates posthumous degree requests for undergraduate and professional degrees for UW-Seattle.

Posthumous Degree

Awarding Posthumous Degrees at the University of Washington (Seattle)

The death of a University of Washington student is a loss shared by the full University. We extend our condolences to the family and friends of the student, as well as to the academic unit and all impacted UW community members.

For students who were within two quarters of meeting their graduation requirements, family members will often ask for consideration of a posthumous degree to be awarded.

University Policies for a Posthumous Degree Request

A degree may be awarded to a student who dies prior to the completion of degree requirements if the following guidelines have been met:

  1. Request is made by student’s family to a UW school official;
  2. The UW school official initiates the UW Posthumous Degree Request form UWNetID;
  3. Office of the Vice President for Student Life verifies proof of death and relationship to family;
  4. Student was within two quarters of graduating;
  5. The Dean of the degree-granting college approves the awarding of the degree.
  6. For graduate students the request also requires approval from the Dean of the Graduate School

If the request is approved, an annotation of POSTHUMOUS DEGREE will be entered on the official transcript and the Office of the University Registrar arranges for a diploma to be created and provided to the family, per their instructions. The words “Posthumous Degree” will not appear on the diploma.

The Posthumous Degree policies and request form described on this webpage applies to undergraduate and professional students at the UW Seattle campus.

Posthumous Degrees for UW Campuses

Refer to the following links, forms, and policies for UW Bothell, UW Tacoma, and the Graduate School.

Posthumous Degree Request Approval Process

  • Family of the student reaches out to a UW school official to inquire about the awarding of a posthumous degree.
  • The UW School official who was initially contacted gathers information from the family including student information, contact information, relationship to the student for the letter that accompanies the diploma, and the address to which the diploma should be mailed.
  • The UW School official submits the UW Posthumous Degree Request which is routed to the Office of the Vice President for Student Life. The review process will have them verify the information gathered by the UW official.
  • The Office of the Vice President for Student Life works with the family to obtain proof of death and connection to the family. The Office of the Vice President for Student Life routes the UW Posthumous Degree Request to the Office of the University Registrar (OUR).
  • The OUR verifies that the student was within two quarters of earning their intended degree and routes the UW Posthumous Degree Request to the dean of the college or school from which the student will earn a posthumous degree.
  • The dean of the college or school sponsoring the awarding of the posthumous degree, indicating approval of sponsorship, routes the UW Posthumous Degree Request to the OUR.
  • The OUR will award the degree and order a diploma. A letter will be created under the University Registrar’s name to be sent with a letter of condolence to the family at the address indicated.

Dean’s List Name

Dean’s List Name

A student’s Quarterly or Annual Dean’s List Name does not have to match their Student Record Name. Students may submit the Dean’s List Name of their choosing using the following form, and doing so affects:

  • Quarterly Dean’s List webpage listing
  • Annual Dean’s List webpage listing
  • Local Newspaper Announcement notifications sent from the UW News Office.

Student Acknowledgment

Students who provide a Dean’s List Name different than their Student Record Name acknowledge:

  • The name will be posted exactly as typed.
  • This name will only be used for posting on the Annual Dean’s List webpage, Quarterly Dean’s List webpage, and notifications from the UW News Office to home town newspapers.
  • Students may not use this feature for purposes of fraud or misrepresentation.
  • The University reserves the right to disregard a requested name if it contains inappropriate or offensive language.
  • Dean’s List Names are posted on this website using the Latin alphabet.

Dean’s Lists

The University of Washington recognizes students after each academic quarter and year for their high scholarship award and inclusion on the corresponding Dean’s List. A suitable and official notation of this achievement is recorded on the student’s University transcript. Students may also be eligible for other Baccalaureate Honors.

Dean’s Lists Criteria

Quarterly Dean’s List

To qualify for the quarterly Dean’s List, a student must be pursuing their first undergraduate degree and attain a quarterly GPA of 3.50 or higher in at least 12 graded credits.

Annual Dean’s List

To qualify for the Annual Dean’s List, a matriculated undergraduate student must be pursuing their first undergraduate degree and have achieved a quarterly GPA of 3.50 in 12 or more numerically graded credits each quarter for three quarters of the academic year (summer through spring).

Students enrolled for four quarters of the academic year (summer through spring) must satisfy the conditions previously outlined and attain a quarterly GPA of 3.50 or better in the fourth quarter, if enrolled for 10 or more credits.

Dean’s List Notations

The official University record that a student has qualified for the Quarterly or Annual Dean’s List is the notation on the student’s UW transcript.

Dean’s List Name

Student’s Quarterly or Annual Dean’s List Name does not have to match their Student Record Name. By submitting a Dean’s List name request via the Dean’s List Name form, students can now have the name of their choice reflected on:

  • The corresponding Dean’s List Recipients webpage listing
  • Notifications from the UW News Office to the student’s hometown newspaper.

Dean’s List Name requests must be submitted prior to the publication of Dean’s List Recipients. Students who do not provide their Quarterly or Annual Deans List Name via the form will have their Student Record Name posted on the corresponding Dean’s List Recipients webpages.
